Anyone may call Alternative Hospice with a request for information. New client information may be provided by the client, the caregiver/family, healthcare professionals from acute care facilities, skilled nursing facilities, assisted living communities, physicians offices, or any other agencies. Alternative Hospice is a physician directed, R.N. coordinated, program of care that responds to the physical, emotional, and spiritual needs of the terminally ill adult... and family. Our Director of Clinical Services assigns an Admissions Nurse to meet with the client / family/caregivers to discuss Alternative Hospice services, review physicians' orders, and conduct initial assessment of eligibility for services.
During client's admissions visit, Admissions Nurse meets with client and family to plan services and develop end- of- life care plan. Client and family are encouraged to ask questions about the disease process and to express fears and goals regarding client care. Empower with information and education reinforcing the Alternative Hospice philosophy, clients complete a thorough initial physical assessment. Durable Power of Attorney for Healthcare and Advance Care Planning information is made available to all admissions.

Once a client is admitted to our program , Alternative Hospice team members collaborate to offer true holistic healthcare choices. True holistic healthcare is allopathic healthcare and complementary therapies used together, as a whole, determined by the individual needs of each client.

Hospice clients must have a terminal diagnosis as documented by qualified physicians. Client declines, refuses, or not eligible for aggressive allopathic care. Now seeking empowerment and guidance to accept comfort instead of cure. Holistic services are offered in homes, assisted living and skilled nursing facilities, hospitals,or any other agencies within our service area. Alternative Hospice team members strive to serve as guardians of peaceful passing in all respects. Empower clients to acknowledge and explore end-of-life desires, we facilitate each client's final journey with dignity and grace.
Hospice is covered by Medicare/Medicaid in most states, and by most private health insurance programs. Alternative Hospice can assist you in determining if your insurance provider will cover care.
