Hospice Medical Director

• Provides consultation to the hospice team and other physicians regarding hospice care
• Co-certifies client's terminal diagnosis with the attending physicians

Attending Physician

• Certifies client's terminal diagnosis
• Coordinates care with interdisciplinary team

Nurse (R.N.)

• Assessment of client and family/caregiver needs
• Provides client-appropriate nursing services
• Coordinates care with attending physicians, hospice team, primary caregivers and/or any other healthcare agencies

Licensed Massage Therapist

• Offers therapies to address physical, emotional and spiritual needs
• Provides individualized client care
• Provides direct access to individualized family/caregiver care

Home Health Aid

• Provides direct personal client care
• Provides comfort measures
• Provides emotional support to client and family/caregivers

Chaplain

• Assessment of client and family/caregiver spiritual needs
• Provides individual family needs consultation to community clergy of all denominations
• Provides direct bereavement counseling and support

Social Worker

• Assessment of client and family/caregiver emotional, social, spiritual, and financial needs
• Provides community resources
• Provides direct counseling and bereavement support

Volunteer

• Provides compassionate companionship
• Provides client/family/caregiver support
• Provides needed non-medical services
